The Nigerian international, who had a decent performance for the host was later substituted in the 62nd minute for Brenna Johnson.
Renan Lodi put Nottingham Forest ahead in the 50th minute with a fine finish before Jesse Lingard extended their lead in the 57th minute with a sublime finish to send the host to the next round of the competition.
However, Emmanuel Dennis was conspicuously missing in action as he was on the bench for the entire duration of the game.
In the other game, Super Eagles midfielder, Joe Aribo was in action as Southampton edged Sheffield Wednesday via penalty shootouts to also book a berth in the next round of the Carabao Cup.
The Nigerian international came off as a substitute for Theo Walcott in the 60th minute after an impressive display.
Sheffield Wednesday netted the opening goal in the 24th minute through Josh Windass before James Ward-Prowse leveled parity in the 45th minute via a penalty.
However, Southampton sealed their place after winning 5-4 on penalties.
